You must sign this “Agreement” and ‘Waiver & Release Form of Liability” to participate in Veritas Christian Homeschool Group.
Once you sign this form, you will be directed to your class registration.
Dress must be honoring and respectful. If the need arises, Leadership will gracefully approach individuals not in compliance. Please use wisdom with words or graphics. Classified weapons (knives, toy guns, swords, etc.) are not allowed on campus with the exception of a possible class project.)
My signature below indicates that I have read and agree with this Statement of Faith and that I agree to adhere to the Dress & Safety Code policy outlined on this page. (Parents are responsible for sharing this code with students. Parent’s signature confirms this has happened and their registered student(s) understand the code.)

All tuition is due in full when you register for classes. However, if you wish to spread the cost of tuition over several months, you have the option of a payment plan.
ALL registrations are “ON HOLD” until payment is made.
Please Note: Failure to attend a full semester does not absolve registrants from their financial obligations to Veritas.

ALL 1st-semester payment plans must be paid in full by October 1st. ALL 2nd-semester payment plans must be paid in full by March 1st.
Co-op Assignment Duties: As we are a cooperative, working together is what keeps us functioning. One parent from each family is required to help fulfill co-op duties on campus. Most of the duties involve being a parent helper to a teacher in one or two classes. Over the summer, teachers may be contacting parents of registered students in order to find class helpers. Those who do not have a volunteer position by August 30th will be assigned a position unless they have opted to drop off their student for the day.
**Some Co-op Assignments are more involved and may require time off-campus, for these bigger assignments, we offer discounts to help offset tuition. These assignments are limited, if you are interested in one of these let us know!
Drop off arrangements: Though we encourage parents to be on campus, we realize that this is not always possible. Please be on time to pick up your student.
Nursery & Preschool: We do not offer a nursery or preschool at this time. All young children must remain with a parent. When you register, please let us know if you will be bringing a little one, we need to have completed medical release forms on file for every child on campus.
Class Fees: The price for classes are shown on the class schedule and are per semester, per student. When you enroll a student in more than two classes the price drops $10 for each class you add.

A Note About Grades: Grade reports will be issued for all core classes for 7th grade and up. You will receive these reports after each quarter from the class teacher. Remember: You remain the primary teacher.
Teaching: We realize the costs of co-op classes for a family can be daunting. Prayerfully consider the possibility of teaching to help offset tuition. You decide what class you want to offer and how you want to teach.
In consideration of being allowed to participate in any way in VERITAS CHRISTIAN HOMESCHOOL GROUP, INC. related events and activities, the undersigned:
1. Agree that the member/participant should inspect the facilities and equipment to be used, and if the member/participant believes anything is unsafe, he or she should immediately advise supervisor (advisor, manager, etc.) of such condition(s) and refuse to participate.
2. Acknowledge and fully understand that each member/participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions, or negligence but the action, inaction, and negligence of others, or the condition of the premises or of any equipment used. Further, there may be other risks not known to us or not reasonably foreseeable at this time.
3. Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death.
4. Release, waive, discharge and covenant not to sue VERITAS CHRISTIAN HOMESCHOOL GROUP, INC. its affiliated clubs, their respective administrators, directors, agents, and other employees of the organization, other members/participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors or premises used to conduct the event, all of which are hereinafter referred to as “releases,” from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury, including death and damage to property, caused or alleged to be caused in whole or in part by the negligence of the releases or otherwise.
To medical facility – As the parent(s) and legal guardians of the minors listed in our family’s registration, I hereby authorize the holder of this form to act upon my behalf in the immediate medical care of my minor child(ren), listed on the reverse side of this page, in the event of an emergency situation which all reasonable effort to reach me have failed.
